Healthcare Navigator

GENERAL DESCRIPTION OF POSITION
Under the supervision of the Community Services Director, the Healthcare Navigator will provide services, including connecting Veterans to VA health care benefits or community health care services when Veterans are not eligible for VA care, and providing case management, care coordination, linkages, health education, and administrative duties.
Key Responsibilities
Act as a liaison between the SSVF grantee and the Veterans Administration or community medical clinic, and work with a population of Veterans with complex needs who require assistance accessing health care services or adhering to health care plans.
Conduct assessments of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members, and significant others to understand the Veteran’s situation, potential barriers to care, the causes, and the impact of such barriers on the Veteran’s ability to access and maintain health care services.
Serve as a resource for education and support for Veterans and families, and help identify appropriate and credible resources and support tailored to the needs and desires of the Veteran and assist them in communicating their preferences in care and personal health-related goals to facilitate shared decision making of the Veteran’s care.
May participate in the development of the Veteran’s care plan; reiterates provider recommendations using clear language to support the Veteran and family members or caregivers and assists Veterans in identifying concerns or questions about their treatment or medications to develop open communication with the provider or treatment team.
Provide comprehensive case management and care coordination across episodes of care—the incumbent acts as a health coach by proactively supporting the Veteran to optimize treatment interventions and outcomes.
Modify services to meet the needs of Veterans best and coordinate services with other organizations and programs to assure such services are complementary and comprehensive; directs activities to maximize effectiveness, efficiency, and continuity of care for Veterans; provide case management services to Veterans serves as the liaison to VA and community health care programs, and represents the program in contacts with other agencies and the public.
Coordinate referrals to VA, community health clinics, and other programs needed to ensure access to health care. The incumbent follows the care plan to facilitate adherence and collaborates with community providers to maximize the use of VA and community resources.
Adhere to ethical principles about confidentiality, informed consent, compliance with relevant laws, and agency policies (e.g., critical incident reporting, HIPAA, Duty to Warn).
Required Skills and Abilities
• Strong verbal and written communication skills and the ability to interact professionally with staff, visitors, and community partners.
• Excellent organization, planning, and prioritization skills with minimal supervision.
• Proficiency in Microsoft Word, Excel, PowerPoint, and email; familiarity with databases.
• Strong attention to detail and ability to maintain accurate records.
• Ability to work effectively in high-pressure or time-sensitive situations.
• Knowledge of general office operations and community resource awareness.
• Initiative, sound judgment, and the ability to make independent decisions.
Requirements
• Master’s level Social Worker or equivalent
• Minimum of two years of experience in a Healthcare or Social Services area of practice.
• Valid driver’s license, clean MVR, and current auto insurance required.
• Must be able to travel occasionally, including some nights.
